Description

RN: Essential Job Responsibilities:

  1. Obtains patient consent for care. Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations. Updates patient information in the EMR.
  2. Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions by answering their questions and requests.
  3. Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
  4. Participates in multidisciplinary teams to improve patient care processes and outcomes.
  5. Works in satellite offices on a rotational basis as needed.
  6. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and/or participating in professional societies.
  7. Complies with all quality assurance policies and Occupational Safety and Health Administration (OSHA) regulations related to safety, cleanliness, and infection control. Maintains strict patient confidentiality.
  8. Promotes a professional environment conducive to optimal patient care.
  9. Performs other duties, responsibilities, and functions as directed by the Practice Administrator, Director of Clinical Services, or Provider.

LPN: Essential Job Responsibilities:

  1. Assists with providing patient care. Obtains, verifies, and records health history of patients. Gathers pertinent health data through physical assessments. Performs tests. Provides basic patient care as ordered. Administers medications and treatments as directed.
  2. Assists other health care professionals in performing exams, diagnostic procedures, and treatments as needed. Places phone calls to pharmacies to order prescriptions; sets up referral contacts.
  3. Maintains patient charts; answers phones; performs other general office duties as required. Promotes well ness by providing educational materials and communicating physician instructions.
  4. Delegates and/or directs patient care activities of unlicensed personnel as appropriate.
